FWD x foodpanda : Terms and Conditions of FWD Health & Go 12-month individual membership redemption
1) This “FWD Health & Go” Membership Program (original: FWD Wellness Pass) (“Program”) is provided and managed by FWD Life Insurance Company (Bermuda) Limited (incorporated in Bermuda with limited liability) (“FWD”). This Program is a membership program but not an insurance contract. Terms and Conditions apply. For details, please refer to the Terms and Conditions of this Program.
2) This Program is not a life insurance product and we don’t ask questions about your current health or any medical conditions. Member must be aged 18-100 and possess Hong Kong Identity Card.
3) The FWD Health & Go 12-month individual membership is valid to redeem until Dec 31,2026 after which it will be null and invalid.
4) The FWD Health & Go 12-month individual membership can only be used by the relevant registered user and it cannot be changed, returned, exchanged for other items or redeemed for cash.
5) Only one FWD Health & Go 12-month individual membership can be redeemed for each customer and can be redeemed at FWD Health & Go website (https://health.fwd.com.hk) by inputting redemption code “foodpanda”. If customer has ever purchased or redeemed any membership (including main and affiliated memberships) of FWD Health & Go, the above 12-month individual membership can’t be redeemed again.
6) The FWD Health & Go 12-month individual membership offers access for only one main member to enjoy the wellness and medical services and no affiliated membership would be offered.
7) Should there be any breach of the terms, fraudulent or abusive activities detected, FWD reserves the right to amend these Terms and Conditions and any terms and conditions relating to foodpanda at any time without notice. In the event of dispute relating to the Program, FWD decision shall be final.
8) For the record of redemption (including but not limited to the date and time), FWD’s system record shall prevail. FWD will not be responsible for any delay, loss, error or unrecognized situation due to computer and / or Internet’s connection, technical problem, malfunction or accident. 9) To facilitate this purpose as stated in clause above, FWD may transfer, disclose, grant access to or share the customers’ personal data with data processors and service providers within or outside Hong Kong. Customers have the right to request access to and, following such access, to request correction (if appropriate) held by FWD. Any of such requests for access and correction should be made and emailed to fwdhealthngo.hk@fwd.com.
10) FWD reserves the right to amend, suspend or terminate the redemption and to amend the relevant terms and conditions at any time at its sole discretion without prior notice.
11) FWD reserves the right to interpret these terms and conditions. In case of any dispute, the decision of FWD is final.
